Taiwan vocabulary vs characters only?

Characters only: 软件 → 軟件. Taiwan vocabulary also swaps in Taiwanese terms: 软件 → 軟體, 信息 → 資訊, 打印 → 列印. For readers in Taiwan, choose Taiwan vocabulary.

Does formatting change?

No — only the characters change.

Are one-to-many characters handled?

Chosen by phrase and common usage, which is usually right; check names and places afterwards.

Why question marks in a converted .txt?

That will not happen: output is UTF-8 with a BOM, recognised by Notepad and Excel.

Is anything uploaded?

No. The dictionary downloads to your browser and conversion happens locally.

Web pages or PDFs?

This handles Office files and plain text.
